Registered Nurse
Grand Junction area • Full-time
Provide skilled nursing care in patients' homes across the Grand Junction area, focusing on wound care, medication management, education, and coordination with families and other disciplines.
Schedule Expectations
Standard weekday schedule with occasional weekend rotation based on patient needs.
Pay Range & Benefits
Competitive hourly rate, mileage reimbursement, paid time off, continuing education support, and a supportive clinical team.
Minimum Requirements
Current Colorado RN license, reliable transportation, CPR certification, and at least 1 year of home health experience preferred.
Home Health Aide / CNA
Montrose area • PRN/Part-time
Support patients with daily living activities, mobility, transfers, and personal care in their homes, working closely with RNs and therapists to deliver coordinated care.
Schedule Expectations
Flexible PRN and part-time shifts available, including mornings, afternoons, and occasional weekends.
Pay Range & Benefits
Competitive hourly rate, mileage reimbursement, training opportunities, and a supportive care team.
Minimum Requirements
Current CNA certification, CPR certification, reliable transportation, and a background check.
Physical Therapist
Grand Junction & Montrose • Full-time
Deliver physical therapy services in patients' homes, focusing on mobility, strength, balance, and safe discharge planning across both service areas.
Schedule Expectations
Standard weekday schedule with travel between Grand Junction and Montrose territories.
Pay Range & Benefits
Competitive salary, mileage reimbursement, continuing education support, and a collaborative interdisciplinary team.
Minimum Requirements
Current PT license, CPR certification, reliable transportation, and at least 1 year of home health experience preferred.

Who can make a referral to Starlight Home Health?
Referrals may come from physicians, hospitals, discharge planners, case managers, rehab facilities, senior living teams, Veteran support teams, community organizations, patients, or family members. Some services may require physician orders, eligibility review, and authorization before care can begin.
What information should be included in a referral?
Helpful information includes patient demographics, diagnosis or reason for referral, requested services, discharge date, physician orders, medication list, insurance or payor details, and any safety or equipment needs.
What happens after a referral is submitted?
Starlight reviews the referral, confirms next steps, contacts the appropriate partner or family contact, coordinates required orders or authorization, and schedules care once the referral is accepted and requirements are met.
How does Starlight Home Health ensure care quality?
Our team follows evidence-based practices, conducts regular patient assessments, collaborates with healthcare providers, and continuously monitors outcomes to ensure personalized, high-quality care tailored to each patient’s needs.
